Recognizing the causes and solutions to nurse turnover is a significant challenge for healthcare administrators. This is known as turnover, when nurses leave their jobs for whatever reason, including retirement, career changes, or personal preferences. Research findings show a direct link between managing nursing turnover and high-quality patient care. It is important to keep that in mind as you calculate the direct costs of replacing a nurse and the indirect costs of poor care due to high turnover (Dechawatanapaisal, 2018). Common causes of turnover include nurses’ feelings of underappreciation, staffing shortages, and the mental and physical strains of the job. Constant turnover among nurses negatively impacts healthcare providers and their patients by eroding trust in one another and making it more difficult to communicate.

To conduct the largest study of its kind in the United States, NSI Nursing Solutions, Inc. contacted more than 3,000 institutions in January 2022. By 2030, the healthcare industry is expected to have grown by 16%, creating over 2.6 million new employment opportunities (US Bureau of Labor Statistics, 2022). The vacancy rate for registered nurses in hospitals has increased significantly. Now at 17%, up 7.1 percentage points from a year ago, with over 80% (81.3% to be exact) reporting a vacancy rate above 10%, this is an all-time high (Colosi, 2022). The results showed that hospitals still do not agree on which job functions should be included in the turnover rate. The vast majority (59.6%) of organizations disclose all forms of employment status change (full-time, part-time, per diem, PRN, casual, occasional, and much more) in their turnover statistics (Colosi, 2022). The remaining medical facilities only provide permanent and temporary positions.

The global healthcare industry is facing a rising human resource crisis due to high turnover rates among nurses. Many registered nurses (RNs) are abandoning the healthcare industry due to the COVID-19 pandemic, either by retiring early or leaving the field entirely to pursue other interests. This has led to astronomical turnover costs for hospitals. The survey found that to improve their bottom line, hospitals needed to increase their capacity to retain patients, reduce their vacancy rate, improve their recruitment efforts, and tighten their control over labor costs.

According to a report released by Illinois Northwestern Memorial Hospital Report Card ( 2018), Northwestern Memorial Hospital had an RN turnover rate of 18.84%. In this paper, I will collect data that fairly represents the existing and desired state of this problem, suggest a change strategy that will assist achieve the desirable state of this issue, provide supporting evidence for that strategy, take into account the perspectives of other professions, and present the results of my analysis and recommendations for moving forward. Northwestern Memorial Hospital needs to change methods to boost patient outcomes and decrease employee turnover. Other problems in the past that could have influenced the high turnover rates of the hospital’s nurses were medication errors and discrimination in admissions based on age or gender (Kwon et al., 2021). An effective technique for reducing medication errors and increasing the proportion of patients admitted based on age and gender may be to increase the retention of existing nurses. Strategies for keeping nurses in an organization can improve morale, broaden career advancement possibilities, and cut down on unnecessary overtime. Implementing a residency program for nurses has been shown to reduce burnout and boost job satisfaction (McNulty et al., 2022). However, there are concerns about the potential difficulties in establishing such a program, such as regulatory constraints, payment issues, and monitoring and evaluation concerns. One of the most common barriers to implementing nurse residency programs is the cost (McNulty et al., 2022). On the other hand, nurse residency programs save money by increasing the retention of new nurses (Cochran, 2017). Regulatory challenges can be overcome by complying with the relevant rules and regulations when establishing such programs. Monitoring and evaluation should be conducted regularly, at least once every three months (McNulty et al., 2022). Despite these obstacles, nurses at Northwestern Memorial Hospital should be optimistic about establishing a nursing residency program.

Patients lose out on having access to a qualified medical expert when hospitals are understaffed. Executives in the healthcare industry must realize how limited personnel negatively affects patient care. Nurses’ likelihood of staying in their jobs can be increased or decreased by several things. The turnover rate of nurses can be lowered, and staffing levels can be maintained if hospitals take steps to enhance the working conditions of their nurses (Kaddourah et al., 2018). If they are content where they work, they will be less likely to look for a new location. Nurse turnover can be reduced with strong leadership and adequate staffing. Administrators in the nursing field would do well to foster open communication amongst staff members to assess their leadership styles and figure out how to better care for their nurses. Nurses should talk to their managers about how they feel about their workload and what they need from them. The main barriers to effective leadership are the failure of organizational leaders to support nurses and the lack of a structured nurse leader pathway. To overcome these barriers, a nurse manager with the appropriate education is required to effectively manage the nursing turnover situation. If hospitals are to maintain their high standards of care, their leaders will need to collaborate to reduce the number of workplace conflicts that drive nurses away. Pay and bonuses for registered nurses have gone up recently because many people who become nurses are offered financial incentives to take the job. The hospital turnover rate might be lowered by offering bonuses to nurses who put in extra hours or take on more tasks.

Many of the systemic operational, procedural, coordination, and technology difficulties highlighted in the survey have been discussed previously. The COVID-19 pandemic was the final straw for the industry and raised awareness to new heights, although these issues have been growing for years and are already a major concern in the nursing profession. Northwestern Memorial Hospital’s administration has reached a tipping point where they must take immediate action to address the underlying operational issues causing widespread fatigue and attrition. It is common knowledge that bettering a hospital’s operational performance can boost productivity, morale, treatment quality, and patient outcomes. The administrative, manual, and tedious duties and processes that further strain already overworked nurses are usually a good place to begin. Nurses want hospitals and health systems to streamline and automate manual activities to improve communication, coordination, and staff scheduling. The potential barriers to streamlining patient care processes through technology to reduce strain on nurses include resistance to change and the need to comply with HIPAA regulations (Whitelaw et al., 2020). To overcome these barriers, the organization must train clinicians to trust a digital process and avoid breaching HIPAA regulations to maintain a positive patient experience.

Justification of Change Strategies

The annual revenue of Northwestern Memorial Hospital is $1.5 million. Thirty-five people work at Northwestern Memorial Hospital, each bringing in $42,857 in revenue (Northwestern Memorial Hospital Revenue: Annual, Historical, And Financials, 2022). In 2021, Northwestern Memorial Hospital earned a peak of $1.5 million. Northwestern Memorial is widely regarded as a leader in patient care and cutting-edge medical technology in cardiology, gynecology, oncology, neurology, neurosurgery, orthopedics, and solid and soft tissue transplants. Because of the scarcity of health care funding, this article seeks to guarantee that monies are being put toward activities that improve patient outcomes, such as providing the highest levels of safety and quality, rather than mere busy work—errors in drug administration and transcribing occurred due to decreased staffing and a shortage of nurses. Northwestern Memorial Hospital and its divisions can do more to help people by working together across disciplines to boost patient care and satisfaction. Northwestern Memorial Hospital’s interdisciplinary team will need to incorporate interprofessional considerations to implement this strategy successfully.

The implementation and planning process is crucial to improve quality treatment; thus, stakeholders, including doctors, nurses, managers, pharmacists, and members of the IT department, should all be involved. The PDSA model could be used to reduce the high number of RN departures from Northwestern Memorial Hospital. Together, these elements and the PDSA framework will help the hospital reduce its nurse turnover rate and improve patient outcomes. Nurses provide continuous care for patients in hospitals. Because of this, they are crucial in guaranteeing the health and well-being of their patients. Researchers have related many indicators of nurse staffing to better health outcomes for patients. How hospitals describe the level or complexity of care in each nursing unit may account for some discrepancies in reported staffing hours. The high turnover rate of nurses affects healthcare systems worldwide. To lower turnover rates, managers and healthcare organizations must know what treatments work best. The majority of studies found that the intervention increased student retention. Job satisfaction and retention were improved through teamwork and individually tailored tactics like mentorship, leadership interest, and an in-depth orientation. Strong evidence suggests that retention strategies based on a preceptor program model centered on the NGN lasting between three and six months are most effective. Literature also supports a management approach adopted by nurse managers that emphasizes creating a pleasant working environment and treating each nurse as an individual within that setting. Despite the many potential causes of nurse turnover, there is still a lack of high-quality data for strategies to address these issues. Despite this setback, additional research is needed to build on the progress made thus far because many hospitals still struggle with retention and nurse shortages. This report recommends that nurses and research funders conduct controlled trials sufficiently powered to account for relevant concepts to evaluate further the efficacy of interventions proven to be successful in observational or quasi-experimental studies.

The increase in internet use in people’s lives has made addiction specialists and psychotherapists witness an evolvement of new issues, including online sexual conduct like meeting casual sex partners, sexual chat platforms, revenge porn, and excessive use of child and adult pornography. These conducts have developed to the point that researchers have recognized other forms of sex offenders, which involve people spending time online as a substitute for sexual, social, and physical contact. For instance, revenge porn has emerged as an offensive area in which intimate pictures are shared without the consent of the demonstrated person and to cause distress. Leaking of intimate images has been observed to be often performed by ex-partners who intend to revenge following a breakup. It may also occur whereby strangers, relatives, co-workers, and peers circulate images to intentionally cause distress or harm to the victim.

Thesis Statement: The paper explores a reflection analysis on child pornography and revenge porn by addressing the public attitudes and perceptions towards the criminalization of revenge porn. It will also analyze the significance of understanding how gender impacts the perception of revenge porn criminalization and the onset and continuance of online child pornography use among sex offenders.

Theme A: Understanding what are the public attitudes and perceptions toward the criminalization of revenge porn is essential.

  • Writer’s Past Position: From my point of view, public perceptions towards criminalization differ based on the context of the issue. Many individuals may perceive revenge porn as a heinous and gross invasion of privacy, which requires it to be criminalized in all states. This involves posting explicit images and nudes on the internet and social media platforms without the victim’s consent to harass or embarrass the victim. Besides, in many cases, the explicit images may be accompanied by crude comments and descriptions of personal information. The involvement of the descriptions, along with the nude images, often results in physical harassment of the victim. As a result, my perception of public attitudes toward criminalizing revenge pornography is that the public supports laws and regulations that control such behaviors. This involves the public attitudes towards implementing criminal codes across the country that must catch up with culture and technology by criminalizing revenge pornography practices of both those who develop the sites and those who post explicit images and videos without the subject’s consent.
  • Moment of Learning: However, I have learned that public opinion and attitudes may significantly influence the implementation of new laws criminalizing revenge pornography. According to a study conducted by Lageson et al. (2019), it was observed that there is widespread support for criminalizing revenge porn among women and men. Also, the perceptions of criminalization differ across the forms of non-consensual pornography. According to the authors, public support for the issue of revenge pornography is centered on implementing laws against posting explicit images of a stranger or a spouse. There are few public opinions towards laws protecting the subjects who self-produced explicit pictures. Thus, public opinions are based on the subject’s guilt. Hence, provided with the apparent association between sexting and conducts traditionally related to promiscuity, it is conceivable that people who had intimate images exposed because of being involved in sexting can be viewed as promiscuous (Starr & Lavis, 2018).
  • New Position: Having understood different public opinions about criminalizing non-consensual pornography, my new stance on the concern is that many individuals support the criminalization of the conduct. This is because exposing an individual’s explicit photos, especially those captured without their consent, may cause distress and embarrassment to the victim. Therefore, public opinions demonstrate that laws and regulations should be implemented in all states to eliminate the growing issues of non-consensual pornography. However, people who self-produce their explicit photos, thinking they will not leak or be exposed, should be partially accountable for their victimization. Hence, the support for criminalization is lower when the victim has self-produced their images, has a connection with the image disseminator, and can be alleged partially accountable for co-creating the media and their succeeding victimization. This is because people may blame the subjects for their sufferings because of personal factors that contributed to their conduct of self-producing the images (Starr & Lavis, 2018). In this context, people can negatively examine the morals of those who sent the explicit photos, blaming them for their immorality, including why the person took the images in the first place.

Theme B: Understanding how gender impacts the perception of revenge porn criminalization is significant.

  • Writer’s Past Position: From my point of view, gender may influence the perception of criminalizing revenge pornography acts. Women are more likely to support the criminalization of revenge pornography than men. Provided that women are seen to be more susceptible to getting involved in sexting, and considering that females are more likely to be affected than males by revenge pornography based on emotional and social consequences. Women are the most vulnerable individuals to revenge porn and have been victims. In a society where females are provided the sole responsibility of maintaining their dignity, and when there are scenarios of revenge porn, the subjects are usually persecuted and blamed. Circulating and spreading nudes without the victim’s consent damages their reputations, humiliates them, violates their trust, destroys their job prospects, and significantly impacts their political and social standing in society and mental health. Therefore, women being the most vulnerable to revenge porn may influence their perception of criminalizing revenge pornography. Women may seek the government to implement laws that protect them from such bullying more than men.
  • Moment of Learning: However, I have learned that gender may significantly influence the implementation of new laws criminalizing revenge pornography. According to a study conducted by Lageson et al. (2019), it was observed that there is widespread support for criminalizing revenge porn among women and men. The authors also indicated that there is much support among females about criminalization, but they are less supportive of retributory penalties. In their study, Lageson et al. (2019) reveal that an estimation of about 97% of females supports criminalization compared to 90% of males, which depicts a significant difference in gender influence. According to the authors, there is a gender gap in supporting criminalization that continues in cases of photos circulated by a spouse or partners and when the victim creates photos.
  • New/ Current Position: Having learned from the studies demonstrated by scholars about how gender differences impact criminalizing of non-consensual pornography, my current stance on the concern is that women are more supportive of the criminalization of the conduct. Women are more susceptible to revenge porn and thus are more likely to support criminalizing the conduct. This may help to lessen women being targeted for humiliation and embarrassment by their spouses, partners, or ex-partners through the exposure of intimate images on the internet.

Theme C: Understanding the onset and continuance of online child pornography use amongst sex offenders is significant.

  • Writer’s Past Position: From my point of view, the onset of online child pornography began with the addiction to adult pornographic films and images on pornographic websites. Many sexual offenders are likely to be addicted to adult pornography before finding interest in child pornography. Also, people may begin to possess materials depicting sexually explicit behaviors involving a child when they find them exciting and pleasurable. Thus, an individual’s perception of child pornography may influence them to participate in illegal acts when looking for their satisfaction because of addiction to pornography which starts with adult pornography. However, the continuation of online child pornography may be influenced by sexual offenders finding it attractive to possess or observe materials such as explicit images of minor victims. It may appear attractive to the offenders because of the inexperience or innocence of the children.
  • Moment of Learning: According to Steely et al. (2018), they performed a study to examine the onset and continuation of child pornography. According to the authors, it is evident that Child Pornography Offenders (CPOs) demonstrated numerous reasons for starting online child pornography, which included transforming from grown-ups to child pornography. Some of the respondents to the study said that changing from grown-up to child pornography started by observing adult pornography sites. For instance, Simon, one of the participants in the study and aged 44 years and sentenced for possession of child porn materials, reported that he was addicted to viewing explicit photos of girls between six to thirteen years old. This was after his long-lasting addiction to adult pornography. He later preferred images of young females aged eight to twelve because their innocence attracted him. He also indicated that female children made him thrilled, assertive, and excited. Another participant in the study reported that they did not transition from adult to minor porn but started by looking for child porn on the internet. The individual was curious, had self-esteem issues, or was attracted to minor porn. Steely et al. (2018) also indicate that the continuation of viewing child porn is influenced by the offender’s enjoyment, thrill, risk, and instant satisfaction of accessing child porn. Other respondents indicated that they were attracted to the pictures of child victims since their inexperience and innocence aroused them.
  • New/ Current Position: Having learned from the studies demonstrated by Steely et al. (2018), my current position concerning the understanding of the continuation and onset of child porn is because of personal desires. The personal desires include excitement, arousing, and finding it enjoyable to view images of minors for self-satisfaction.

Conclusion

The information is necessary for everyone because it provides an understanding of child pornography and revenge porn by addressing the public attitudes and perceptions towards the criminalization of revenge porn. It also analyzes the significance of understanding how gender impacts the perception of revenge porn criminalization and the onset and continuance of online child pornography use among sex offenders. I may apply this information in the future for reference when engaging in academic or personal research about child pornography and revenge porn in contemporary society.

Each individual’s background, experience, and personality can influence career choices. Is there any particular experience that can significantly impact a student’s career path? This question can be answered from the perspective of experience. Special experiences are also referred to as significant events that people have experienced during childhood, like childhood trauma. Special experiences are roughly divided into three categories: family, study, and accidents/others. These three can be very influential factors and directly affect students’ judgments and choices.

Bronfenbrenner's Ecological Theory

The family can be a good example. Bronfenbrenner’s ecological theory highlights that the mesosystem is the most influential child-centered group that can directly influence children’s development, including behavior and thoughts. (Santa Clara University, 2022) If we take the family as an example, then a particular experience could be a very traumatic family experience the child had, perhaps physical abuse or moving abroad. Based on the mesosystem, these experiences can cause the most immediate long-term damage to the child. Children will be facing their painful memories; there is a high possibility that children will choose to overcome or avoid these memories. Both situations can contribute to a child’s choice of future career goals.

As well as including family or school, painful memories can also be accidental. Although accidents in special experiences are not explicitly reflected in Bronfenbrenner’s ecological system, accidents are one of the factors that cannot be ignored. The proportion of children under 12 entering E.R in 2022 is already 43%, and this does not include other reasons for witnessing a significant accident. (Gabriele, 2022) The database on the graphic would suggest that some children will witness or experience accidents before they reach high school. These situations can also lead to children avoiding or overcoming such painful memories. These can be the basis of subconscious decisions for students to choose a career and try to avoid a particular occupation or relate to specific content.

The above descriptions only include experience, but the job market also shows that background can influence job choices, especially in the region. In addition to a willingness to work, salary is also an essential consideration for students. Students will choose their preferences based on experience, and salary is essentially a filter. (Sheikh, 2017) This can make it more difficult for people from different backgrounds to choose a career that pays well. This can also significantly impact students’ choices due to cultural restrictions. This is also a factor that cannot be ignored. However, the students’ wishes will be the primary reference for their desired goals. If students are more inclined to be realistic, more background factors and other social rules limit their choices.

Regardless of any factors, this will be the basis for people to choose. The reference will not have 100% belief in a particular factor. The ideal ratio would be 50/50 between reality and ideals. It also depends on each individual’s conceptual understanding of the two, but particular experiences do not fall squarely into either of the two concepts. An exceptional experience is a concept that encompasses both. Because special experiences give people subconscious escape or overcome, these are ideal concepts in many cases. This is because this factor does not combine with other conditions to make a choice more biased toward reality. It will be realistic if the particular experience is combined with a condition. For example, by combining a particular experience with one’s abilities, the particular experience becomes a supporting factor.

According to (Bryce et al., 2021), individuals who have experienced trauma are more likely to choose careers that involve helping others in need; that is, they will choose careers that help resolve the abuse that they went through as children. Doing this makes most individuals feel at peace, validated, and in control of their lives. These authors noted that most people would go after careers that fill the gap they felt in their childhood. Growing up, I had a friend who experienced abuse from her guardians. Her parents were addicts, and she experienced physical, emotional, and sexual abuse. This continued until social services intervened after a teacher noticed that she was not doing well and her performance had started dropping. She moved through many foster homes over the years and never had any absolute stability in her home life. These experiences and what she saw other kids going through in these group homes led her to be very dedicated and determined to have a better life for herself, which would eventually lead to her helping kids that have gone through what she did. She attended therapy sessions to help her overcome her trauma, giving her a chance at a normal life. When we got to high school, my friend chose subjects that would contribute to a career in social work or careers in psychology. She was motivated by the social worker who handled her case and kept in touch for years to ensure she was doing well. Eventually, she chose to major in psychology so that she could help abused and neglected children find standing in this world. She also influenced some of us to engage in social work and charity events aimed at helping disadvantaged children in our community. She is almost graduating, and I can confidently say she will be a force to reckon with in her field. Her experiences pushed her and motivated her to become one of the best professionals out there. Studies have shown that more than half of certified therapists and over 30% of mental health practitioners have experienced abuse in their early years. A study on 558 subjects working as therapists and law enforcement officers found that over 29% of therapists and 19% of officers reported having experienced childhood abuse (chart below). It also noted that more women working in the helping profession experienced childhood adversity than women in other fields.

Experiencing a challenging upbringing may make an individual be biased, project, or have less objectivity in their work. However, it may also be a strength; in their quest to find meaning and healing from their trauma, they may become a valuable professional in their fields (Bryce et al., 2021). They may be valuable because they are more likely to have a higher level of empathy and a stronger desire to help others who have experienced similar trauma.

Childhood abuse experiences among law enforcement officers and mental health professionals

Childhood sexual abuse may lead to women feeling invisible, silenced, and not having a place in the world. In my friend’s case, she insists that she will only work in departments or organizations mostly run or led by women, as that is where she will feel safe. This further proves that abuse has long-term effects on the victim and the choices they make in their careers. All victims of this kind of abuse will always look for careers that will offer them the safety that they never had growing up (Reilly & D’Amico, 2008). Another aspect that may have led to my friend choosing the careers she did is the need to right the wrongs done to her in the past. This is a significant motivator for individuals who have experienced adversities in their early years; they mostly want to be involved in fields that ensure they help vulnerable women, children, and families; this gives them a purpose in life.

Childhood adversity impacts individuals’ career choices after high school and university. People want to have meaning and an understanding of their life experiences; therefore, most will choose careers that align with these needs. Childhood trauma lasts a lifetime hence the need to protect children hence reducing these incidences to have well-balanced adults in the future. Children may be undergoing trauma without those around them realizing it; this further shows the importance of education on the forms of trauma and how to prevent it, as it may cause adverse effects on children (Meyers, 2014). There is also a need for child protective services to keep up with what is going on behind closed doors to reduce these instances.

In conclusion, childhood abuse can significantly impact an individual’s future career choices. Thus, it is essential to deal with childhood trauma in order to make better career choices. Dealing with trauma can make individuals more aware of their goals and make better career choices. It is also essential for parents and guardians to educate themselves on the various forms of trauma that can happen to their kids so that they can prevent it and also identify it early on to help their kids get professional help early on. It is essential for adults who have not yet dealt with their trauma to attend therapy sessions to deal with these issues. Every experience and trauma is valid and should not be dismissed; mental health is a real issue affecting our society today, and all the relevant authorities should invest resources in tackling it.
